• have experienced a fever, chest congestion with cough, shortness of breath, other flu-like symptoms in the last two weeks
• have travelled to high-risk areas in the last two weeks (specifically: China, Iran, Italy, South Korea, Hong Kong, and Japan)
• have come into contact with someone diagnosed with Coronavirus/COVID-19
